About The Event

This annual conference serves as a catalyst for positive change and collaboration between community rehabilitation providers, business, state and local government, educators, advocacy organizations, social service agencies, and individuals with disabilities. 

  • Each attendee must register individually. For Gold/Silver Sponsor Levels ONLY,  please use the "Sponsor-Extra Representative" ticket to register your extra allotted sponsor conference attendee(s) registration.
  • Note: Please contact the Osthoff directly for any special overnight accommodations you may need. We are unable to give any refunds once you have registered.
  • Room Registration: Osthoff Resort in Elkhart Lake, WI. Reservations-call 800-876-3399, press prompt #1, and state Disability Service Provider Network, or online, click “Group Login”, enter GDISABLE (Group #), SERVICE (password), and make reservation.

    Sponsor receives two full conference registrations, a full-page four-color ad in the program, recognition in the program book, on the conference webpage and conference banner. It also includes one item from your company for the raffle on Thursday, with recognition of your donation. This option will also be the sole sponsor of the Thursday post-conference reception with recognition indicating your support.
